Our Pediatric Appointments
Well Care Appointments
Please plan to schedule well visits in advance. Keep in mind that we will be busier in the months of August and September due to schools being back in session.
Non-Urgent Appointments
Less urgent issues, such as acne, warts, or more chronic conditions (long-standing abdominal pain or headaches, constipation, behavioral problems, eczema, etc.), should be scheduled in advance.
Sick Appointments
We offer same-day sick appointments to allow our patients to address their illnesses as quickly as possible. We recommend calling early as these do fill up quickly.
Health Forms and Letters
We are happy to complete forms for school, camp, etc. The charge is $10 for physical forms, $20 for other school forms, and $25 for FMLA paperwork. If your child has not had a physical within the last year, please schedule an appointment so that we can complete the form with current information.
Medical Advice
During business hours, we are happy to answer general questions over the phone. For more complex questions, please make an appointment so that we may evaluate your child and provide you with the most accurate information and the best care.
Urgent matters will be addressed before the day is over. For less urgent matters, we will return your call within 24-48 hours.
After Hours/Weekends
For after-hours advice, please contact your insurance company's nurse line.
For urgent medical concerns, we recommend Good Night Pediatrics Urgent Care, Banner Thunderbird Emergency Room, or Phoenix Children's Hospital Urgent Care or Emergency Room.
If you have a concern that needs to be addressed by Dr. Hoff, please call our office and follow the prompts to leave a message. Please keep in mind that this is ONLY for matters that cannot wait until regular business hours.
